ef9345: fix quadrichrome color mapping (#9251) 
The third byte (A) in quadrichrome mode contains an ordered 4 color set,
referred by index in each of the 4 pairs of bits making a character
slice.

Fixed the second PSG addressing on the ECHO+ (#9330) 
The ECHO+ uses a single VIA to address two PSGs. The BC2 lines of the PSGs are connected to PB3 and PB4 respectively. Both PSGs are reset through PB2.
